You just set the program or use manually, it comes with a remote control and it does all the work. It moves her legs for her and she does not complain of being in pain or uncomfortable. I have to strap he feet to the pedals but I made it work. My family and I use it as well. This might be nice for some. But lack of directions started this off on a bad "foot" so to speak. It's also super heavy. A friend came over to get it put together for me not a fun time!!
