The pad can be positioned to adjust tightness of the elastic belt which is a inch or two thinner than most. This allows me to sit or bend over comfortably. Larger belts tended to cover the bottom of my rib cage and interfered with breathing. The dimple on the pad is effective yet gentle. I ordered this for my sister who has tried most every design of umbilical hernia belts--from thin to very wide. BY FAR THIS IS THE BEST! This is the most comfortable width--wide enough to be effective, and if it folds a bit it still works comfortably and well. PLUS the fabric the is most comfortable. For the price, try this one! Update months later: Yes, it's not perfect, but after seeing all the belts she's tried no belt is perfect. It can fold over after hours of wear, but she says it does stay in place for her, is very comfortable and holds tight. Again, for the price, might be worth a try! I do like the padd for pushing hernia thats all i like ..... because.... it moves from place so much i end up taking it off ... its missing some non slip lines and would be perfect. Also elastic is not very firm so feels not very supporting. ..... i had to biy another
