This back brace fits well and is comfortable to wear. I found it to probably be sized larger than other back braces I've tried. I almost feel like I can go down a size and still be able to use it. So if in between sizes, I would recommend going down. As far as support, it has an aluminum panel straight down the lower part of the spine. It is contoured to the shape of the spine. For me, I really couldn't feel it when I wear the back brace. It's really only noticeable when I feel the brace itself. As a result, I felt like I kept needing to tighten the brace more just to get more support. Maybe it would work better for me if it had more panels in the back or along more sections instead of just in the middle where the spine is. I've tried many velcro back braces for myself before, but ordered this one for use with my patients. Based on the size guide, I feel this one does fit True-to-Size. For reference, my waist measurement is 33" over my shirt, so I fall into the Size Small category. I ordered this brace in Size Regular (waist 35"-46") for my patients to try, so it fits a little too large for me. I had to pull the straps of this Regular size all the way to the end of the velcro, and it still needs to be a little tighter on me. I feel this brace gives medium support. I like the metal piece over the spine, and the softness elsewhere. I like that it has 2 layers of straps, so that the bottom layer can be snug, then the top strap layer can really cinch it down as tight as needed. One side has tiny air holes in the material to make it more breathable. The brace uses the newer type of velcro which is smoother, rather than the old hook-and-loop type which becomes picked and pulled over time. However, I wish that the velcro areas of this brace were larger, for a stronger hold/attachment. I removed one star for this reason. I'm not a believer of wearing back braces constantly, as that can make your core muscles weaker and dependent on the brace. And the instructions even advise not to wear this for more than 4 hours at a time. But wearing this brace for short periods can give external support + act as a reminder for the wearer to use good posture and to engage their core muscles, rather than become dependent on the brace.
